I'm 22 weeks and haven't received my MATB1 form yet, didn't really think too much of it but I'm not due to see anyone again until 25 weeks when it needs to be in. I've never met my midwife and it's never been discussed. I sent a message to the midwife team today but have heard nothing back. They've been useless throughout to be honest so I don't expect I'll get a response. I'm going to call my GP tomorrow to make an appointment for 25 weeks and whooping cough. If I mention it could they do one for me? When was I meant to get one, at my scan??

I had the same worry but at my 25 week appointment my midwife wrote one up for me, it’s filled in there and then in my area.

Mine was done at 20w scan and I didn't even need to ask, it was just done as standard practice.

I had to phone up for mine - just phoned my midwife and she posted it out to me.

I called about mine as I needed it before my 25 week appt for work, I just popped in a picked it up. But I was offered it again today at my 25 week appt so that might be standard?

I’ve been told that I won’t get it until 28 weeks as my 25 week is a phone appointment. My employers want it by 25 weeks though so might have to see if they will post it or leave it at the GP surgery! So frustrating.

I needed it at 24 weeks for work so I texted my midwife who left it for me at GP reception - might be worth calling midwife and asking for it if you need it! It's a very simple form of just name, due date, they stamp it and you fill in your NI number and address.

My work needed it before my next midwife appointment, I just called my GP and the midwife left it at reception. I would def make sure you get it on time as it can affect whether you get your mat leave pay or not!
