pregnant after vasectomy

Has anyone here found themselves pregnant after their partner has had a vasectomy. It was a joint decision that we were done having babies, but here I am with a few symptoms and a faint but still likely positive test.

Has anyone found themselves in this situation before?

I could really use some advice

Did he have a sample analysed after his vasectomy? They normally have to check after so many weeks to check it was successful

Yes, both came back clear

Yes this happened to m in 1990s.
We had to proceed with both a termination and another vasectomy.
Nurse was distracted by somehow drawing blood on her finger.
It is extremely unusual but not impossible.
