I have received confirmation of my maternity allowance and on the letter it says that I need to fill in “Your Reply” on page 5 &6 when I stop work. However they have only sent me pages 1-4! There is the return envelope included. I’m dreading having to hang on the line for hours like last time. I’m probably being overly optimistic but does anyone know what this form is and if it can be found elsewhere without me needing to ring up and ask for another form?
