Filling maternity record in?

I got my maternity record in the post the other day with a letter for my booking appointment (this is my first baby) which isn't until next month, the letter advises to fill in as much as possible beforehand but how much detail do they need?

I'm debating putting no to having reading and writing difficulties because mine are quite mild, and having to explain to health professionals what dyspraxia is if I end up having multiple people reading my notes, is taking up their time when it doesn't need to since it shouldn't impact me in labour i think.

Why not leave it for now and then discuss with your midwife at your booking appointment and decide from there whether it's appropriate to put yes or no?

Put in as much as you feel comfortable with, for example on the health conditions checklist I would note any diagnosed conditions eg asthma, diabetes. The midwife will go through it all with you, it just makes it quicker for her to see if there's anything which needs referrals.

@Woodlandwalks yes I ll leave it for now, I don't really feel I need support for it and so I wonder do I need to be spending the few minutes it'd take to explain what it is.

@SagaBauer I will put down the couple of other conditions I have and maybe I ll get a separate sheet to put my family history down on because its a bit long so i m not having to think about it for a while if they ask.
