Help another maternity pay one

Ok so I finally got the right form from my boss to claim mat pay but it says at top of form must be done at least 15 weeks before due date with everything going on and me not having a clue what to do and boss not helping me it's 11 weeks until my due date does this mean I've missed it and won't qualify now ?

Just do it ASAP, I can't see that they would penalise you for this. These policy rules exist to prevent staff from announcing the week before that they are pregnant and claiming pay. Just get it submitted on Monday, I can't imagine it would be a problem.

Thanks it's going straight back tomorrow just worried as I've never applied before and had no help or info from anyone Just glad I finally have the right form.
