Haemorrhoids in third trimester

So I have had haemorrhoids on and off during this pregnancy but since Friday last week (6 grueling days) I have been in absolute agony. I eventually saw the GP today who examined me and said he's never seen them so bad before. I have one very large one that is thrombosed. He referred me to the surgical assessment unit at hospital. My husband took me there and the surgeon wanted to operate but was hesitant due to a general anaesthetic being too dangerous in pregnancy. He examined me and decided I wouldn't be able to handle a local anaesthetic either so he sent me away with some gel and told me to sit on ice. I am now absolutely terrified of going in to labour and having to push with this terrible pain in my back passage. The surgeon suggested I ring my midwife tomorrow and ask her opinion on what is best. Has anyone had them this bad and managed to give birth naturally without further issues? Or what would my options be? I'm UK based.
