Found out I'm pregnant whilst abroad - what now?

I'm currently in Asia and have just (very happily) found out I am pregnant with our second baby. I'm investigating clinics for the first 12 week scan and initial pre-natal appointments, but I'll be back home in the UK for about 6 weeks in December which will tie in with the 20 week scan. Does anyone know how I should go about contacting my GP to get signed back up to midwife care? Should I wait for my 12 week dating scan, email and attach a scan so they can use it for their records when booking me in for the 20 week scan? Although I will be going abroad again at the start of the New Year, we'll be home for good from 34 weeks (or earlier if it is a difficult pregnancy) and will have the baby at home. I'm just wondering how I should go about contacting my GP to ensure I can get booked in for the 20 week scan at the right time when I won't be home for the normal midwife appointments leading up to it. Grateful for any advice! Thank you
