Maternity allowance form query

For anyone who's filled out the form - I'm currently on sick but returning in a couple of weeks. How do i fill the attached out? Since I've not started maternity leave yet? Do I give the dates of my SSP from during the pregnancy or tick no?

It's asking for information during your test period.

@bouncingraindrops yes I know that but the way it's worded 'after you last worked'

Just write the start date of sick and provide additional Info on page 11?
