MATB1 form error

Regarding the matB1 form. How is it written on the form? Is it supposed to be for example 24/08/2019 or 24 August 2019? My midwife has written it as 24 August 2019 but it's crammed in. My HR said I need a new one with the date written as 24/08/2019. Can anyone show me how it's supposed to look before I go back to my midwife? I'm really worried. My midwife is quite difficult anyway. Thanks in advance.

Mine was written as your example so say 30 August 2018. No question from my hr... not sure if mine was wrong but seems like your work is being difficult

I work in HR and it doesn't matter how the date is written on the form. How ridiculous! Sounds like your work are being unnecessarily difficult

I know, it's ridiculous. I'm going to push back to my HR and tell them it's readable. They're both awkward to be honest. It's stressing me out.
