Can anyone help me regarding SMP

I'm 30+5 weeks and my smp still isn't sorted. I sent my boss my matb1 form on the 28th August and nothing has been done or said. I asked her exactly 2 weeks after her receiving it what was going on and she said she has it but hasn't had time to get a copy of it. I haven't given any notice and only just found out i should of given notice at 25 weeks yet i didn't even receive my matb1 until i was 28 weeks 🤔 She has known about me being pregnant since 20 weeks so if all this was needed from me by certain dates why wasn't i told ? Is it too late now ? And how much notice do i have to give before i can leave ? I just don't know what to say to her because I'm clueless in this situation, and i know i will get fobbed off or my text will get read and ignored for days until i have to remind her about it and feel daft. I just want to get it sorted and know exactly what I'm talking about instead of her saying she will find out and not text me back until i ask about it. I get the feeling this is being purposely dragged out now. Sorry for massive post and thanks if anyone has any advice.

You can give notice of when you want to start your maternity leave without having your matb1. The matb1 is for your maternity pay. You should give your notice asap as it should have been done by 25 weeks

The earliest you can start maternity leave is 29 weeks btw
