Midwife had filled in my Mat1 B form wrong

Hi all,

I’m 27 weeks pregnant with my first - my midwife hadn’t mentioned the mat1b at my 24 week appointment and my boss has been pretty rubbish in telling me what paperwork I need to supply (no idea what I’m doing!)

Turns out my employer needs it ASAP so I phoned my midwife and asked her if she could leave it at my GP reception for me, but for some reason she’s put my date of birth in the expected week of birth box!

I’m assuming yhere’s no way I can amend this myself as it’s a legal document. I’m going to have to ask her for another one aren’t I...? 😣
