MatB1 - incorrectly filled out?!

Hi all - received an email this morning from my work about my maternity leave application, querying the dates on my MatB1.

I'm actually seeing my midwife this afternoon so before I go in all guns blazing can anyone confirm what date should be on there?

My EDD is Friday 5th October.
The date on the MatB1 is Saturday 29th September. If I'm correct it needs to start on a Sunday so should be Sunday 30th September?

My midwife does have form for doing this, my notes are covered in crossings out, the worst was when she put me down as due mid August!

Yes I agree with you. It should say 30th September as that’s the Sunday of the week you’re due to give birth.
Hope you can get it sorted this afternoon.
