I've been ringing my midwife for days left messages and need this form by next mon. What am I supposed to do.

Is there a central number for the midwives?might be she is on leave or suchlike and you can call centrally. Are you going to be 25 weeks on Monday?

Just go to your GP. I called mine and they had it waiting for me to collect at reception, didn't even see the doctor! Smile

Do they run regular clinics? MW told me just to drop in on one of their clinic days and collect mine so you could try that if they don’t respond? Good luck!

Yes just call your doctors. I asked for mine from my doctor yesterday and picking it up today.

Agree with others, call your GP. I did this and it was ready for me to collect from reception within a couple of days.

As others have said, call your GP. You don’t have to make an appt. just say to reception and they’ll have it waiting for you a couple of days later.

There really is no point getting stressed out about it, your midwife could be off sick or on leave. Call the GP surgery or the Antenatal clinic at your hospital and ask if they can help you.
