anyone know about uterine bleeding in second trimester?

my friends had her first scan (at 16 weeks foe some reason) for her second dc today and was told she had a small uterine bleed. Has this happened to anyone else and what does it mean?? thanks.

did she notice she had bled, ie did it come out of her? i would have thought if her doctors did not take any action , it is probably ok....as long as baby and placenta are ok, i would try not to worry !
