Mat B 1 form filled incorrectly?

Hi i'm due to see my HR manager today to give in my matb1 form. After filling in my part and photocopying I realised a few things on the part the midwife fills in which seem off. Firstly on the section where it asks the date before or after confinment she has left blank. Do I fill this in??
Also on the date of examination section she has filled in the babys due date and date of signing as the date i saw her. Is this right?
Thought I would ask incase I need to cancel meeting and arrange to see anothet midwife (mine is off sick for a month)

Doesn't sound right to me... I'd arrange another appointment to check it.

She either needs to have filled in ‘date of confinement’ which is when you had your baby if she was doing it post-birth or estimated due date. If I remember correctly, due date should be on the top right of the form - you can fill this in yourself if she’s left it blank. Then on the bottom left there is date of examination and date of signing, they will probably be the same.
