Do I ask my mw or is there a form to fill in? Need to take advantage of my free dental care!

ask MW as there is ap rt they have to complete

Just ask at your gp surgery, the reception normally have them or ask your mw, i got mine at my booking in appointment.

ask your midwife and she will fill the form for you or you'll have to do it yourself

Ask your midwife - there is a form for them to sign and for you to complete and send off.

I only just got given the form yesterday (I'm 32 weeks - so no one can really argue about it!) You need it as soon as possible.
