**Handing in MATB1 form**

I've been to my doctors today for my MATB1 form, he has totally screwed up my details so I'm unable to hand it into my employer.

I have booked an appointment with my midwife for Tuesday 9th January (Due to the new year I couldn't get an earlier appointment).

I have read online I should tell my employer about my maternity leave adleast my 6th January. Will handing in my form a few days later effect me a lot?

I was planning to still write to my employer with my maternity leave dates and hand in my MATB1 as soon as I get it.

Have any of you's had this problem?

What has the Dr got wrong? You cannot have two MATB1s - I tried to get another as I have two jobs. Can't you contact the Dr and say they've got it wrong and amend it?

I would tell the employer in person and an writing (email), contact the HR and tell them the form is coming, and what will be on the form. My form was late and doing this was ok.

You should be able to phone your gp surgery and explain and they'll get it sorted for you!

I handed mine in way too late, at 34 weeks-ish, so I'm sure you'll be fine.

Your duty is to inform your employer by the 15th week before expected childbirth. So you just need to tell them. As pp said in person and in writing is best.

Then give them the form when you get it. I don't see why you need an appt tho. Women have just turned up to see the midway fe before appts and got the form at my doctors.

Mine were happy to be told in writing and to pass on the form when I received it (technically 2 weeks late)
