Baby's 2nd name dilemma

So my baby's dad doesn't want to be put on her birth certificate but I wanted my daughter to have his second name and mine, like a double barrel. Can this still happen even if he's not on the birth certificate? Or does she have to just have my name?

You can name dc anything but will have to leave df details box blank.

Why not have his surname as a mn if appropriate?
Many /most dm with useless dc df regret giving dc his surname.

Without trying to offend but why would you want her to carry his name if he doesn't want to be on the certificate? Confused
