Mat1b form - do we need two?

I'm sorting out my Maternity Allowance (self employed) and the form is clear that I need to send the original Mat1b form with my application form.

My husband is wanting to apply for paternity leave and shared parental leave through his employer (NHS) and says his work thinks he needs to send of my Mat1b too.

Should his employer accept a copy/seeing the original and getting their own copy? Or do we need a second form?

I had a similar issue as I have 2 employers. I have to say it was pretty difficult as the midwife was adamant I could only have one, and each employer wanted to see the original, and one employer wanted to keep it. I did a bit of reading up, and found out it is right that you only get one, but there is no requirement for your employer to keep the original. So I would go with showing each of them the original and getting them to take their own copy.

You only get one so they will need to give you the original back, it's a good idea to make a copy just in case.

I did a copy for OH for his employer that was accepted.

Just as well since DWP didn't bother to send the MatB1 back

The midwife will only issue 1 but the NHS accepted a copy for my husband's paternity leave.

I was wondering the same for maternity leave but also registering with a dentist for free dental care!

Your free dental case is via the NHS Exemption certificate which you get a different form to send off, my midwife gave that one at my booking in appointment as you need that way before the Mat form for your employer

Ahh thanks DebsOctBaby. I'll ask midwife at my 25 week appointment in a few weeks- it hasn't been mentioned to me at all but then I live in Wales and I was going to have to wait until 25 weeks to get my MatB1 so had to request it early for work, maybe it's just different over here?

mumma I'm in Wales and got a funny look when I asked about an exemption certificate, so I don't think we have them here. The midwife said the dentist should just accept my notes as proof of pregnancy, but actually they didn't ask to see anything.

We had this with DP's work. They were 'demanding' my MATB1. When I checked there is actually a kind of self cert form for fathers - www.gov.uk/government/publications/ordinary-statutory-paternity-pay-and-leave-becoming-a-birth-parent-sc3
Have a look at the guidance too as it could be quite handy!
