Form for bloods - is this taken at 28 week midwife appointment?

Im confused....i have my whopping cough vaccine this week (will be almost 21 weeks). Then i need to make 25 week appointment with GP but i remember midwife saying something about me needing this bloods form....but i am not seeing midwife again til im 28 weeks. Do i need to book in?

Mine were just taken at my 28 week appointment (or rather they tried to take them) without any forms.

I had to do bloods again at 28 weeks, but I'm not sure what you mean about a form? I didn't have to fill a form in but maybe it's just procedure at your GP/midwife. Don't panic though, it's normal and to check your iron levels etc.
