Hi,
Can anyone tell me who you get this form from. Is it your GP? Its just that my pregnancy is classed as high risk and my care if totally hopsital led so don't see anyone about the pregnancy at my GP's surgery. I would ring them first to ask but they are totally useless on the admin side so thought I'd ask on here first. My the way, I'm 21 weeks.

Ok, so the hospital midwife should be able to give it to me? Its just that when I asked for the free prescriptons form at the hospital they said I had to get it from my GP so I wasn't sure if this form was the same.

well mine was from gp midwife, but if all your care is carrie dout in hosp(ie youve had no visit to doc midwife) then yes hosp midwife should have it..hope you get sorted
