Hi ladies. When did you get this form, who from and did you have to request it? I'm almost 23 weeks and haven't seen anything

You should get it after your 20 week scan, speak to your midwife the next time you see her.

Thanks for your reply. I'm not due to see midwife until 15 March and my work need it before then or I won't be entitled to enhanced mat pay. Any idea who I should contact?

Contact your midwife anyway, she should be able to write you one out. You need it for work by week 25 I think? Its amazing because every pregnant woman needs this form, I don't know a single person who didn't have to nag for theirs!

I got mine done at my hospital appointment but believe I could ask at GP reception and they'd get it signed for me to pop in and pick up. Might be worth giving them a call.

Btw my employers waited for the form when I handed in my maternity information, they just needed it sent in after I got it.

Your employers will need to see it but don't need to have the actual form by then, although you may have to have applied for maternity leave by then.

They've requested it by then and have said they need it for EMP so not going to risk it! I can't apply for maternity without it.

Just phoned my doctors and it should be ready by Thursday. Thanks for everyone's responses really helpful :)

My midwife told me to text her when I'd had 20 week scan and she'd leave me at the GP to pick up

GP can do one for you or just call your community midwives.
We give them out at 28week appt as first time we see women after 21week scan. If you need it earlier just call.....
