Hi ladies just a question. I'm 23 weeks now and entitield to milk tokens, sent of my exemption certificate, I recieve them from sending this or apply separately??

You must fill out a healthy start form in order to receive the vouchers. These are available to print/obtain online or may be able to get one from your midwife depending on the area you live. Act quickly though, as I believe they don't back date anymore

Ok thanks I know a health care professional has to sign the form, can this be a doctor as my next midwife app isn't for another 5 weeks? Thanks x

I believe it is any relevant medical professional, and as a doc has access to your med notes and can confirm "how pregnant" you are it should be ok
