Had a scan today (I'm only 4+6) due to health problems.
The sonographer asked do I have any abdo pain (which I do) and said I've free fluid? Have to come back in 2 weeks and see Dr today at 11am.
What's free fluid? Don't want to google.

The most common reason would be an ovarian cyst that's burst.. But they will want to exclude other causes, such as retained blood from a previous failed conception or an ectopic... They'll want to check you out a bit more to see exactly what it is. The scan in two weeks will likely be to check up on the gestational sac itself - 4.6 is too early to see much but by 6 ish you can often see a heartbeat.

Free fluid is (unhelpfully, sorry) fluid that's not contained in a structure. So for example if you'd been in a car crash they might scan you to see if you were bleeding internally into your pelvis - that'd be one cause of free fluid (not the case here of course, I think you'd know about it if you had a broken pelvis ;) ) Things like burst cysts etc can be another source and are really common. I had a whacking great one that hurt like hell until it burst...
