Matb from GP?

Argghh i have an appt with my gp and was going to ask for my matb at the same time, v hard to get antenatal clinic appts here but have left my hospital notes Confusedat work and cant get them in time for appt. Can my gp dp form without? Will notes from hospital be on my gp records?

I'm an "employer" and all of my pregnant ladies have got this form from their midwife?

Yes would ask a midwife but hard to get appt and didnt get at scan. Thanks for replies x

Midwives give them out at the 25 week appointment, usually.

I just called and requested mine and the midwife left it at reception for me to collect

I got mine from my gp at my 22 week check up. But as the others said, if you phone in and ask you can usually pick it up at reception :)

Also you don't need to hand Mat B1 in until something like 12 days AFTER you go on maternity leave so there's no urgency! As long as you let work know in writing 15 weeks before what date you are going
