Can you get a matb1 form from your gp or does it have to come from your midwife?

Hi I got mine from my midwife when I was around 20 weeks, photocopy this and keep a copy! If you are in desperate need you can go to the gp.

Hellymummy · 16/05/2014 15:16

Think it can come from gp. I got mine from hosp at 28 weeks when I went for my anti d jab!

I think GPs can issue them too. Why do you need it before your next MW appointment?

Both can provide you with MATB1.

I asked my midwife for it and she sent me to my GP at around 20 weeks. Then on my next appointment at 28 wks she asked me if I need one Hmm

Thanks ladies, have an appointment with go next week and I don't see midwife between 16 and 34 weeks as I have been told I have to see gp and consultant instead

Assuming you are somewhere between those dates then, bear in mind I think you have to be 25 weeks for them to issue it. Smile

my work asked for MATB1 to be on my file no later than 25wks, my GP appointment was at 24 wks and I got it then, no problem.

Your pregnancy is confirmed with your second scan (around 20-24wks) so as soon as this is done you can ask for MATB1 (GP or MW)

PenguinsHatchedAnEgg · 16/05/2014 16:33

Sorry, yes, double checked. The earliest they can issue is 20 weeks. Many areas say they won't until 25 ish.

Your work can ask for it by 25 weeks, but they can't make you meet that deadline IIRC.
