I'm 25 weeks tomorrow.
Had 24 week appointment yesterday and forgot to sort out the form. Called doctors surgery this morning and they said that I could pop in and get a form.
DH popped in on his way home to pick up a form for me, and the receptionist reluctantly gave it to him as the midwife needs to sign a load of stuff on it.
I don't know why they said on the phone I could get one if it's no use to me without the midwife.
My next appointment is 13th may. Can I wait til then to get the form signed or should I book an appointment for next week instead? Midwife is only there on a Tuesday. Can a doctor sign it instead? Then I have more flexibility with times and days. I work FT so is a pain. I've already got to go for GD blood tests next week so will need the morning off work. Agh if only I'd have remembered.

I was told I could ask the midwife at my GTT to fill in my matb1 x

Thanks. Where roughly are you in the world?
When I had earlier blood tests it wasn't in the antenatal part of the hospital, so I don't know if there will be anyone there to sign it. I'll ask them and go from there I guess?

I called the doctor reception and asked them to ask the midwife to fill in the form and leave it at reception for me. She did that and I picked it up as I was driving past so didn't need to be there at the same time as the midwife. I'm not sure why yours are being so unhelpful - what good is it to you if it's not filled in? Hope you get it sorted.

Any GP at the practice can sign too. All they sign is a box to confirm your EDD, not lots at all. Sorry you've been fobbed off. Good luck getting it sorted.

Thanks both of you. I'll call the doctors tomorrow and ask them if they can get my doctor or get the midwife next Tuesday to fill it in for me and collect. If not I'll try and get an appointment with the doctor. Thanks again.

Have you no direct contact for your midwife? I just rang mine when I forgot to get it at my 25 week appt and she left it for me at the docs reception.

I have got a number for her but last saw her when I was 9 weeks. Since then I've seen various other midwives. So could I message her even though I haven't had any thing to do with her since booking appointment? I've no idea what her schedule is like or if she attends any appointments at my surgery because for each appointment I've booked it's been someone else.

There's not a lot for mw to sign for on matb1. It's just a bit of paper saying that you've been seen by health care professional ( midwife) and she confirms that you're pregnant and your EDD date is ...

Have a look at your pregnancy folder OP, there should be a contact phone number for midwives, give them a call.
