What do I do? I really can't remember. Do you still have to write the letter which states your EDD and date which you wish to start maternity leave. Roughly when did you all hand that in?

Someone more professionally informed than me will be along surely, but as I remember I told my boss, then HR. I had a meeting with HR where they asked for my MatB form and an idea of when I'd be finishing (although that can change). I don't remember putting anything in writing.

Do you have a staff handbook? You have to tell them 15 weeks before Edd at the latest I think and provide copy of the matb form.

I just got given a sicknote from the doc to say i needed reduced hours as it was physicly demanding job, and walked up to mine to tell him lol he wernt happy but i dint give a toss....thought he could sack me at first :) then he called me back to say different x

I'm lucky enough that I work for a small business where it's just me & the boss. Would dread to think about all the paperwork involved with HR... Good luck!
