Lost my notes

And I had been doing so well till now :(

27+6 and cant find my notes anywhere, all my midwife contact details were in there as well, feeling stupid i didn't save their number in my phone
And i either had an appointment last Thursday or today which i have missed

Do the hospitals/midwife have a copy and who would i need to contact? either my Doctor or the hospital?

Just contact your community midwife, they can advise you what you need to do to get new notes. Everything should be on the computers anyways. Don't worry about it, I'm sure it happens a lot!
