Midwife writing incorrect/false info in notes

Anyone had any experience of this??

She's written I'm generally feeling well - umm no I told you I can barely move due to spd and I am back at the physio and on the strongest does of pain killers I can have...!

Uterous size - she didn't even touch my stomach let alone measure it..!

Apparently my bp is high at 107/70??

And a couple of other stupid comments - shall I phone & complain and see someone else? Or just write to pals

Maybe they put the opposite if the truth means more work for them.

My BP is high at 130/85 so I am monitored very regularly. What I wouldn't give to have your BP during this pregnancy! You are not at risk of pre eclampsia with a BP like yours. I would definitely complain.

Ullapull Thank you. I've actually started re-seeing my old psychologist who looked after me when my OCD was very bad, and he thinks I'm fairly low risk for PND. I'm lucky to have family who will pay for me to go private though!

Mine did this when I was pregnant with dd. She said the scales weren't working and how much did I weigh? I said I didn't know so she said I looked about 11 stone. We also discussed my appointment next day for a full scan to make sure the baby was ok.

At the scan, afterwards, the nurse was fiddling with the paperwork and results and I asked her to explain how she worked it all out. She said she used my weight as a guide etc etc. I pointed out that the Mw had guessed my weight. She was quite annoyed and weighed me again to find I was 10 stone and she redid all the stuff as the Mw info was wrong and could have caused her to read the scan results wrongly.

She told me she would report it, no idea if she did, I saw the gp after that.

I never experienced this myself (had brilliant midwives) but have heard of it happening. I am a HCP in another specialty and write patient notes all the time.

It is NOT acceptable to write outright false information and it is a serious probity issue. There are no excuses. It is one thing to be distracted and tick a box accidentally, or make an honest mistake; entirely another to pretend she has measured your uterus when she hasn't.

Report to the supervisor of midwives.

Hi, I went in to maternity assessment for reduced movement. Thank goodness everything was fine but about 3 and half weeks later at a growth scan a urine sample had to be sent off to the lab. This made me check my notes. I noticed the urine sample I gave at that visit for reduced movement came back as Leucocytes and Protein + (which I print screened as my notes are on an app) but I had never been informed. Anyway when I rang to find out the results of the sample that had went to the lab which were luckily negative, I questioned this and the woman was rather shirty and didn't give me a straight answer, pretty much just said well it doesnt matter because your result this time was negative. My trust was starting to disappear as I felt like I should have been informed. At the next midwife appointment (31 weeks) I asked again about this. She just said it mustn't have been enough to be a concern. At that I decided to drop it, after all me and my baby seem healthy. However, at 32 weeks, 4 days I decided to have a look at my records again. The record from my visit with reduced movement from when I was 26 weeks now says NAD for urine sample (which I have also screen shotted). This to me is evidence that my records have been altered and I am not happy about that! Why would they alter my records???!! What should I do? Am I being bothered by something that doesnt really matter as me and my baby are healthy or am I right to be annoyed that my records appear to have been tampered with??!!!

As a midwife I wouldn’t jump up and down about a plus of protein and leukocytes if everything else is normal. That suggests that there is vaginal discharge in the sample more than anything.

@Bienentrinkwasser, I am not jumping up and down as I know I am healthy. What I don't understand is why the results have been changed a month later. My BP recorded has changed too. It may not be important information now but it could be for someone else. I just don't get how results of tests can be modified a month later! Surely changing results is not the norm.

Okay totally not a big deal but in my maternity notes my midwife said I had an epidural with my last pregnancy. I didn’t . I don’t know why it annoys me lol . I’m just leaving it though because who cares lol But I secretly do care and I don’t know whhhhhy ??!!!

@Lavenderhoney wow, that’s odd she would just guess. How can anyone guess someone’s weight ?? I have actually never been weighed for any of my pregnancies. They just asked me my weight at my booking appointments and I told them . But I knew my weight so that’s different.

In both my pregnancies at the booking appointment the MW has ticked boxes for a load of stuff we did not discuss but I haven't really been too concerned about that. When it's results etc I would be concerned. Funnily enough my labour notes from last time said I laboured in the pool when I definitely did not... I had a C-section at all of 2cm dilated 😑 wish I'd had the pool I'd planned on having 🤣

The midwife I saw at booking at the hospital ticked the box to say I was a heavy smoker. I don’t smoke.

I was very confused when I got a call from the stop smoking service to offer me support. When I said that I’d never smoked the woman got really huffy and told me not to lie, how important it was to be upfront and honest with them for my babies health.

When I pointed out that my carbon monoxide test in my notes was 1 and that wouldn’t be possible if I was a heavy smoker she backed down.

Saw the consultant at 20 weeks, again he started saying how I must stop smoking in the lead up to my c section and he’s going to refer me to a consultant anaesnatisit to go over the risks of smoking before surgery - all while I’m sat there saying “I don’t smoke! I have never smoked in my life!”

Christ on a fucking bike.

The booking midwife also put my Dd birthweight in wrong world it looks like she was on the 9th centile at birth which means I have to have repeated growth scans for this baby.

She wasn’t, she was on the 50th. I’ve showed the consultant her red book as proof. Got the answer “but it says on your booking info....”

I’ve emailed the supervisor of midwives to get it all changed on my records.
