What is it? I was give this by my midwife today but I dnt know what it's for?

It's to give to your employer so you can claim maternity pay :)

If you are working you need to give it to your employer by 25 weeks pg for them to process statutory maternity pay.

If you aren't working you don't need it, unless you are going to claim maternity allowance if you are entitled to it where you might need it, not sure.
