When did everyone receive their mat1b form? I'm twenty weeks this week and my work are asking for it.

Do I need to chase my midwife or will I get it at my scan next week? Other than that not seeing midwife until 24 weeks.

From memory it was at about 24w appt but you can just ask.

Also I think your GP can issue it so you might be able to ring up your surgery and get them to do it for you to pick up without needing an appointment.

Same here - think it was around 26 weeks. MW just gave it to me, but you should be able to ask them at your next appointment.

My MW gave it to me at my 24w appointment. You only have to give it to work at 24 weeks.

You don't have to hand it in to your employer until 24ws, mine were chasing for ages before that though, really got me flustered. I picked mine up from the midwife when I was 23ws.

Hi Op on the actual MATB1 form it does say MW can't hand it over till after 20 weeks. Legally you don't have to provide to your employers until 15 weeks prior to your due date. I gave my employer mine at 24 weeks. If you ask your MW she should be able to provide it at your next appointment.

I rang the midwife when I was 20 w and she wrote it and left it at reception for me. As I was having DC3 the appointments didn't seem that regular and I didn't want it to get forgotten or missed (by me!)

I got mine at my 24 wk apt. Didn't give it to work straight away as had problems with sending it to the right department and my smp has been paid fine.

Mine had a strop when I went in to ask for it at 24 wks! Front desk made me book a full appt (which wasn't due for another 4 weeks.) Midwife said that I was wasting her time and that I should have gone to the GP for the form. Surely they would be equally busy? All quite stressful as I was potentially being made redundant. Also very confusing, as the pregnancy notes state that your midwife should give you the form at 20 weeks.

My MW said they don't usually issue them until the 25 wk appoint, but I had to ask for it early (after 20 wks) as work wanted my application for maternity leave with form no later than 15 weeks before my due date. And I thought that would be cutting it fine.

So if you need it, just ask. They should be fine :)
