It would appear that my MatB1 form has been 'lost' by my HR dept.

Hmm Helpful.

They say that they are 'going to have another look around' for it- which doesn't instill me with much confidence.

I have a photocopy (which I took for DP to send in for his paternity)- they will have to use this surely if they can't find my original?

I'd've thought they'd have to make do with the copy as they're the ones who lost it? Maybe they should take more care with important pieces of paper.

Im a bit pissed that I would have to make arrangements again to go and get another one. I don't live on my Dr's doorstep. Its a right PITA.

The same thing happened to me (except I hadn't made a copy). I calle my midwife and she popped another one in the post with a 'copy' stamped on it but all the original dates. Don't stress xx

Must be down to individual midwives as mine said that she only ever issues one MATB1 form and that she advises all her mums (to be) to make a copy of it so in the case of work 'losing' it, they would only receive a copy to replace it.

My employer lost mine, I Just got another one from the midwife.

Your dh does not have to provide any evidence of your maternity other than a declaration from you if he is taking additional paternity leave. If an employer is asking for evidence you can refer them to the legislation.

Your hr department do need a MATB1 or other medical evidence signed by your doctor or midwife confirming your due date. They need this to claim back SMP from HMRC. Only one MATB1 form is usually produced and any subsequent should clearly be marked DUPLICATE.

How annoying for you that they have lost it!

happened to me too!! my hr are fking useless twats! my line manager made copies for me just in case but the cheeky cunts say they wont take a copy and 'suggested' i hand deliver it to the office and get a receipt!! not happening as its too far a walk for me to manage from my office to theirs.
