MAT1B form

I'm 26 weeks and have just realised I haven't been given my MAT1B form yet. My employer knows I'm pg and are very chilled - they aren't chasing it, but at some point I'm pretty sure they'll ask!

When should I have been given it/who by? I had a midwife appt last week but it wasn't mentioned, and I have got another in a couple of weeks (when I'm 28 weeks).

Just phone your mw and ask her to fill you one in, I texted my mw after my 20wk scan (as she told me to) and then just picked it up from the surgery the next day x

I got mine at my 25 week appointment, I had a reminder on my phone to ask her about it but she actually brought it up herself. Your employer knowing is the important thing, but you should give them the Form at some point, just ask her for it at your 28 week appt, my midwife had a pad of them with her and they take 2 secs to fill in. Good idea from pp to text her before perhaps to remind her.

I just rang Mw and she left it at the surgery for me at 24 weeks
