Can't read midwifes notes - please help decipher!

I have my 12 week scan on Thurs and am just reading through the notes from my booking apt 3 weeks ago as not sure when next apt is meant to be after that. The notes are pretty much unreadable though from what I can make out it says 'nuchal fold & combined test arranged 20/40' then a squiggle I can't make out. Underneath there are further squiggles ending in mw @ 16/40. I presume this means I need to make an apt with the midwife at my docs (which is a different one) around 16 weeks but I thought the nuchal fold test was done at 12 weeks and I don't know of any apt having been booked yet for 20 weeks. Is the 12 week scan and nuchal fold one and the same?
Also find it a little weird I seem to see a different midwife every time and then give birth in a hospital that none of my apts have been at, with midwives I've never met. Is this normal?! Thank you

yep normal to give birth at a hospital whilst having antenatal care in community. That is standard. The midwives you meet in delivery you won't have seen before. Many people often see different midwives for different appts - depends on your location. I tend to see one of 2.
The 12 week scan will include a nuchal fold measurement but plus or minus bloods (combined test) - this depends on your local NHS. This pregnancy for me it was all combined at 12 weeks, last one in different county we had nuchal fold and 12 week scan on NHS but they didn't combine with bloods so we had this done privately. A google should tell you what your health authority does.
They would usually book your 20 week scan at the time of your 12 week scan.
You will need to make a midwife apt at 16 weeks. The front of your antenatal notes should have info on appointment frequency.

you may find for future appts at your GPs you see the same midwife more consistently.
