Midwife said I won't get until my 28 week appt (other than 20 week scan I have no more mw appt until 28 weeks as it is second pregnancy so they don't care see you less often.

But my work and dh's work want matb1 form as soon as possible.

Can I get it from my GP? And from what week?

Yes my schedule says 28 weeks if it's not your first. Workplaces don't need it till 28 days before due date, I've been told.

Yes your GP can write you one.
From 21 weeks, I believe you can collect it.
I just called my midwife and she left it at surgery for me to collect.

I got mine at 22 weeks - I agree with seabuck :)

I asked my midwife at 16 week appt as like you, wasn't seeing her again until 28wks and work wanted it asap (I didn't trust them not to lose it as well if it was sent over separately to my formal notification!).

I just had to phone her at 22wks and she completed it whilst I was on the phone left it at the reception desk at clinic for me to collect that afternoon.
