Im 17 weeks and still not got mine. Grr. I'm sure I had it long before now with DD. how do I chase this up?! I'm not seeing the midwife till 24 weeks so I can't speak to her until then. Any advice ladies??

I really have no recollection of filling in a form with DD, it just arrived one day, I don't think I was even 12 weeks.

Sigh.. Something else to add to the 'to do' list ..

My GP gave me the form at first post-conception appointment and told me to send it in once I had an EDD from the hospital. If you have to get a prescription from the pharmacy, definitely tell them you're pregnant. I hadn't even sent off the form yet didn't have mine but the pharmacist filled the prescription for free. I'm guessing they send off the green slip to somewhere (probably too advanced to expect a database)?

It comes v quickly after sending the form. I lost mine and reported it missing and got another within a week.
