pregnant again and i have a 5 month old baby - absolutely scared!!

my little girl is turning 5 months old on the 27th of this month... on pancake day i found out im pregnant only 5 weeks but im so scared... my ideal weight for my hight is 12 stone and im 16 stone so im scared im to over weight that something will go wrong or my body hasn't go over the birth experience i had with my little girl as my body nearly gave up, also i had aweful morning sickness first pregnancy and lost two stone im scared of missing out on things with my little girl if i have that bad morning sickness again, this pregnancy wasnt planned we think condom split but not sure :/ im just scared something will go wrong???? im already drinking loads of water and swimming 3 times a week with a friend who is already pregnant just to get my fitness and health back up, also i have postnatal depression and im worries after this baby will i be worse or ok - im not sure as i not long found out i had postnatal depression and been put on tablets... things where starting to look up with them, ive got doctors appointment next week to talk about the tablets and being pregnant and these tablets are not ment to be stopped straight away..... but ive started taken the multi vitamins for pregnancy through out. i dont no weather to book my 1st midwife appointment at 10-11weeks as im scared if anything bad happens i will have to cancel further appointments this way i can have my 1st midwife appointment then go straight for a scan the next week? im only 20!!! 21 end of this year and will have two kids! me and my fiance was also thinking of getting married this year but i dont know if it all feels rushed now. it all hasent sunk in yet and feels abit unreal... i started my driving lessons up so fingers crossed time im heavily pregnant i'll be passed by then....... im all over the place... can you tell????? lol
