When do we have to hand this form in by? Is it 25 weeks? Or do you just have to let your employer know in writing by then, and hand your form in later?

My next midwife appointment wont be until I'm 24+5 so really pushing it if it's 25 x

I'm 27 weeks and I was given one this week.

Check your work maternity policy - because of baby brain, I forgot to pick up my MAT B1 at 25 weeks so had to wait until 38. I told work and I only had to give them written notice with the form, 28 days before I intended to start maternity leave, so was ok. We'd already agreed leave dates verbally by then, so just needed to put it all in writing. Hope you have a similar policy (I'm not sure if 28 days is the legal requirement or just my own company's policy)

Thank you. I'll try to phone head office tomorrow (I work in a shop so it's sometimes difficult tracking down the right person.)

I have also phoned my MW and asked for my form, so hopefully she can leave it at the surgery for me x

25 weeks but I got mine today at 21 weeks just to get it all done early and get my dates sorted. Can see your midwife anytime to get one.

I saw my midwife last week but I was only 19+4 so she wouldn't give it to me then. Hopefully she can now x

I'm not seeing my midwife till i'm 25 weeks (now 22 weeks) so I rang her up and she completed form and left it at surgery for me to collect.
My work had been bugging me since I was 16 weeks for it even though I told them on more then one occassion I won't recieve it till I'm 20 weeks so dare not be late with it LOL

Hi i had awful issues with my form last time so i made a midwife appointment at 21 weeks this time and got it then, handed it in then. i think it was sent off when i was 28 weeks.

You should be able to inform your HR dept and tell them form is to follow but you must inform them by 25 weeks. Had to nag midwife for mine.

My maternity policy said they needed it by 25 weeks and it HAD to be the original and they couldn't send it back to me either unless they photocopied it.

One phone call to HR and they were pretty flexible about it. Didn't get the form myself till 26 weeks. DH needed the original for his work to sort paternity so they had it for a few days so my employer didn't get it till 27-28 weeks. Keep um in the loop and they'll be fine

My work haven't been the easiest so far though, they've been asking me to proove that I'm pregnant since I told them at 10 weeks (due to heavy lifting). I've just phoned and left a message with the midwife so hopefully she can do my form for me. xx

My firm states they must have it by 25 weeks (along with confirmation of my intention to take maternity leave) but as this is my second baby I am not actually seeing the midwife again until 28 weeks so cannot get it before then. Have told work verbally and in writing that I won't have it until 28 weeks and that seems to be ok.

My firm has to have it by 25 weeks, maybe it varies from place to place.

The MW won't give it you until 20 weeks. I had to bring forward my midwife appointment a couple of weeks to get mine in on time!
