Maternity exemption cert - who sends it off?

Hi, I was just wondering who sends off the form once the Dr has signed it. My Dr didnt mention it at all at my appointment and it was just out of luck I remembered (14 weeks now and I have regular meds each month) So went down to the surgery to fill in a form about a week and a half ago. I thought the Dr's would send it off or do I need to go and pick it up and post it myself?

First time I filled it in and midwife did rest, last time I forgot so midwife filled it out and left it at reception for me, I filled it in a posted it.
This time I have forgotten again will get one at 25 weeks now I think (if I remember) DS's is still valied at the moment so no rush for me.

Think mine took a couple of weeks to come through, so it may have been sent, and in the system.

they posted mine. all i did was fill in my part.

I had the form off my midwife signed by her on behalf of my Gp I think,but I had to send the paperwork off myself only took a couple of weeks for me.

Midwife signed her part at booking in - I filled in my part then had to drop it in to doctors surgery for doctors signature and they then posted it off for me

Both times mw gave it to me at first appt; she signed it and put in EDD, I did the rest and posted it. Was back in less than a week.

Mine forgot all about it, was only when I needed a script for a rash that I remembered. Reminded her at the 21 week appt I think.

My midwife signed the bits she needed to then I filled my bits in and sent it off.
