MATB1 form ?? Shouldnt I have it by now ?

I'm currently 23+4 and it occured to me today I should probably get a move on and fill in my maternity leave application form and let people at work know my intentions, how I want to be paid, when im leaving etc etc. Then I realised that I now don't have a MW appointment until I am 28 weeks now and I don't have my MATB1 form yet. Aren't I supposed to have it by now ?
What should I do ??

You can get it from your GP.

I found I had to ask - it's not something that was offered to me automatically. In fact I asked for it at my 20 week scan, when I was 20 +5 - and the midwife refused to give it to me as you have to be 21 weeks apparently. 2 days early FFS! I got it 2 days later from the midwife I saw as part of a research project I am taking part in. Even she thought that was a bit tight.

My mw asked me if I needed one. I guess she thought since I have two DC already I probably wasn't working.Hmm I got it at my 24 week appointment

You don't need to give it to your employer to 15 wks before your due date so 25 wks, I had to ask m/w too at 26 wks for it so they don't seem to offer it automatically which is weird if they know u are working.

I had to remind them i needed mine by contacting the GP, i picked it up at 25+4.

I was given mine at my 25 week midwife appointment.

In my area you don't get it until 25 week apt unless you specifically ask - I had to go collect mine from midwife at about 23 weeks.
