Does the FW8 form only cover free NHS dentists? I have toothache and can't really afford to see my dentist otherwise.

Yes I think so - we were talking about this at NCT and one of the girls said she wouldn't be getting free dental treatment as she goes private. Nothing to stop you signing up to an NHS dentist but of course you have the waiting list! The form exempts you from consultation fees and treatment fees so I'm getting all my dental work done over the next year!
