MATB1 Certificate - How do i get it?

Went for my 25 week MW appt and it was sooooo rushed (2 mins if that) and in the whirlwind i didnt remember to ask. Do i just ring the docs? Theres no way i'll ever get through to the MW as she never answers the phone and theres no answer machine.

I did the same thing!

Either the doctor or midwife can complete the HCP part, so just call the doctors and get them to complete it and either leave it for you to collect or ask them to post it to you.
