Apologies for ignorance.

You will need it to support your claim for maternity pay/allowance (and therefore give it to whoever will be paying you. Will your former employer be paying you? If not you need to get a Mat Allowance application form from the JobCentre.

You give it to your employer to prove you're entitled to company maternity pay.

You say previous employer, are you not working now?

Your midwife will need to fill in your due date etc, then give to your employer after you have filled in when you want to start maternity leave. The earliest you can start maternity leave is 11 weeks before your due date. HTH

Gosh, I am really confused.

I stopped work end of July but my contract was up on the 1st of September (teaching contract) and so am entitled to the £106 a week.
Do i now send this form to my employer (previous as my contract was up a few days ago) and they deal with it from there?

I'm really sorry for being so thick

I would talk to your previous employer - from the information given I wouldn't be certain as to whether you are entitled to SMP or Mat Allowance (they're both £106 per week). But your employer will be able to help. From memory you have to go through your employer as they have to respond as to whether you are entitled to SMP or not.

Just to make it more confusing...

If it's Maternity Allowance you're entitled to, I got the form from the local Social Security office, as the Job Centre denied all knowledge of it. Correspondence has subsequently come from the Department of Work and Pensions - although the return address remains the social security.

It's taken a while to get mine, but that's been because of problems with the Inland Revenue self-employment people. The Maternity Allowance people themselves have actually been quite efficient.

Thanks for all your replies.
I have just emailed my employer to clarify what exactly i'm claiming and the next step.
I had just assumed that once i had this form that was it...duh!! Hope this is not a reflection of how I am going to be nearer baby's arrival time..
