When to send my SF100 form.

I believe I am eligible for the sure start maternity grant and was wondering what is the earliest date/week I can apply. I am currently 26+4, due date 6th Dec. Tia x

Midwife is probably the best person to ask.

According to the above page your can apply from your 29th week of pregnancy.

You can apply from 29 weeks, if you phone up the jobcentre they can post you out a form. The back page needs to be filled out by your midwife and think they need your MATB1 form.

I sent my form back with my maternity allowance form and they sent it all back saying I hadn't sent them a MATB1 form. When I phoned and complained they had it and must have lost it they said they send the SF100 form and maternity allowance forms to different parts of the country and they'd sent my MATB1 form with my SF100. So I have to get another form for my maternity allowance form - which is nearly impossible as the midwife can only issue you one! So make sure you photo copy everything for your own records in case you have any problems!
