Hi, I sent off for mine a couple of months ago but haven't had anything back. Wondered if anyone knew if there was a number I could call to find out what's happened to it?

You need to speak to your GP or midwife I think, normally they send it off for you. Are you sure you sent it?

Hi sanielle, my midwife gave it to me to send off. It did have the surgery details on it though so maybe it was sent back to her direct. I'll check! Thanks for your help!

I gave my form to the receptionist at the doctor's surgery and the card came through after a week or two I think.

I just called the number babynelly2010 gave me and they were able to find me on the system. Turns out they sent it to the wrong address, so they're resending and I'm all set!

I have been waiting for 7 weeks.

Doc said they are talking months with some of them (to anyone else who hasn't received theirs).

Ass, they haven't received mine, but the doctor had it in her in tray on the 7th July.

They were really quick with mine (like 2 weeks or something) so I reckon it's lways worth giving them a call if you havent had it and need it.
