I had my 28 week check up yesterday but forgot to ask for my HiPG form - is there any other way I can get it?

No, it has to come from the midwife or GP. They didn't have any left when I went for my apt so I ended up driving to the birthing unit and getting one because I didn't want to wait. However it was paid today (less than a fortnight later) and the only restriction is that you have to send it off within a certain timeframe of it beng signed by the midwife.

Can't you phone your m/w and ask for one? We've got a line which is open each morning for an hour or so, and is manned by a couple of the town's community m/ws, for general queries and advice.

Or else phone your GP and ask?
