Not seeing midwife - how do I get these forms?

I'm not seeing the midwife at all, so how do I go about getting a prescription / dental treatment exemption and also a MATB1 to confirm to my employer I'm pregnant. My GP is aware and I'm booked in and have consultant apps. I would have though t a GP could just issue it, but have tried just ringing GP, but receptionist very unsure and said "ask the midwife"....

GP should be able to provide them. The MATB1 form isn't given till 25 weeks though.

i got my prescription exemption form through the gp as they have to sign it and send it off.

GP should be able to do it. Says on the form they can. Consultant too if you have an appt coming up.

my gp did my exemption form I didn't even have to see them ( obviously after the initial referral visit) I filled in the form left it at reception the doctor signed it and sent it off for me.
The matb1 form I would try and ring the antenatal clinic your consultant is based at and ask them what to do you might find they book you an appointment with a midwife for you matb1 form and your health in pregnancy grant form ( if you can still get it)

my high risk clinic has a midwife attatched to it.

my consultant also used to have the ability to refer me to a midwife.

my GP did mine in my most recent pg, but consultant did it first time round. didn't even need to see my GP, just filled in the form and left it at reception for the prescription exemption.

I got mine from consultants clinic. I don't see a MW either
