And I am shitting myself, he is six years old, a couple of times he has been ok but the last few times it has really stressed him out,

Getting him into the pet carrier is a job in itself but once we get to the vets he completly freaks,

He is such a gentle lovely boy at home, he still pads and licks me, my clothing, even a quilt if im in bed, anything that is close to me,

Last time when the vet went to give him the injection he went for the vet, she jumped back and demanded he had a towel was put over his head, I just feel so bad putting him through it but I know I have to,

He didnt come near me or the kids for days after,

I would suggest asking the practice if they have a vet who is particularly good with nervous cats. This is my area in our practice, I personally find that most of these cats dislike to much handling and the less we do the better they are. I also spend a couple of minutes winning their trust and in particular try to encourage them to come out the crate themselves rather than having to be hauled out.
Also try getting a little bottle of Feliway spray and spraying some in the carrier 15minutes before he goes in to help him feel less anxious.
