Important letter. Anyone willing to check my grammar/punctuation?

I’m currently receiving treatment under the care of a consultant. It is not going well and I’m trying to compose a written request to have my care transferred to another consultant.

Would anyone be up for checking my grammar and punctuation? I don’t want to come across as a total numpty.
